Effects of Proper Palmar Artery Island Flap Combined with the First Metacarpal Radial Dorsal Flap in the Repair Treatment of Finger Amputation with Soft Tissue Defect

Abstract:Objective To observe the effects of proper palmar artery island flap combined with the first metacarpal radial dorsal flap in the repair treatment of thumb and finger amputation with soft tissue defect.Methods Retrospectively selected 64 patients with thumb and finger amputation complicated with soft tissue defect admitted to the First Affiliated Hospital of Xinxiang Medical College from January 2021 to January 2022 and divided into two groups according to different treatment methods.Thirty-four patients in control group were treated with proper palmar artery island flap repair,and 30 patients in study group were given proper palmar artery island flap combined with the first metacarpal radial dorsal flap repair.The flap survival status,clinical therapeutic efficacy,ranges of motions of injured interphalangeal joints[proximal interphalangeal joint(PIP)and distal interphalangeal joint(DIP)]and hand function[Michigan hand questionnaire(MHQ)]were compared between the two groups.Results All flaps survived in both groups and all patients were followed up.The excellent and good rate of finger function in study group was higher than that in control group(P<0.05).At 1 year after surgery,the ranges of motions of PIP and DIP of injured side in both groups were greater than those before surgery,and the ranges of motions in study group were greater than those in control group(P<0.05).The scores of items of MHQ scale in both groups at 1 year after surgery were better than those before surgery,and the scores were better in study group than those in control group(P<0.05).Conclusion Proper palmar artery island flap combined with the first metacarpal radial dorsal flap has a significant effect in the repair of thumb and finger amputation with soft tissue defect,and it can effectively promote the recovery of finger function and interphalangeal joint range of motion,and improve the hand function.
