Effect of Different Posterior Fixation and Fusion in the Treatment of Scoliosis in Children

Abstract:Objective To explore the application value of lumbar fusion intermuscular pedicle screw fixation with different approaches in the treatment of degenerative lumbar scoliosis complicated with lumbar spinal stenosis,in order to provide clinical basis for the treatment of such patients.Methods A total of 86 children with scoliosis who underwent posterior fixation and fusion in Zhengzhou Orthopedic Hospital from February 2020 to February 2022 were randomly divided into two groups,43 cases in each group.The observation group received long segment fixation and fusion,while the control group received short segment fixation and fusion.The operation and postoperative conditions,preoperative and postoperative inflammatory indexes[interleukin-6(IL-6),procalcitonin(PCT),tumor necrosis factor-α(TNF-α)],imaging indexes(lumbar lordosis angle,Cobb angle),spinal function[Oswestry disability index(ODI)score,Japanese Orthopaedic Association(JOA)score]were compared between the two groups,and vertebral fusion and vertebral recurrence were followed up for 1 year.Results The postoperative ambulation time and hospitalization days in the observation group were shorter than those in the control group(P<0.05).The serum levels of IL-6,PCT and TNF-α in the observation group on the 7th day after operation was lower than that in the control group(P<0.05).At 3 months after operation,the lumbar protrusion angle in the observation group was greater than that in the control group,and the Cobb angle was less than that in the control group(P<0.05).At 3 months after operation,ODI score of the observation group was lower than that of the control group,and JOA score of the observation group was higher than that of the control group(P<0.05).There was no significant difference in the vertebral fusion rate and the recurrence rate of scoliosis between the two groups one year after operation(P>0.05).Conclusion Long segment fixation and fusion and short segment fixation and fusion are equally difficult in the treatment of scoliosis in children,and both have good long-term efficacy,while the former can improve spinal morphology,restore spinal function,and reduce inflammatory reaction.
