Effect of Different Doses of Dexmedetomidine on Stress Response and the Levels of Reproductive Hormone in Patients Undergoing Benign Prostatic Hyperplasia Surgery

Abstract:Objective To explore the effect of different doses of dexmedetomidine on stress response and the levels of reproductive hormone in patients undergoing benign prostatic hyperplasia surgery.Methods A total of 153 patients with benign prostatic hyperplasia were divided into group A,group B,and group C using a random number table method,with 51 patients in each group.The group C received high-dose dexmedetomidine,the group B received low-dose dexmedetomidine,and the group A received 9 g·L-1 sodium chloride injection of equal volume.All three groups were observed until the patient was discharged.The sedative and analgesic effects and cognitive function 4 hours and 1 days after surgery,stress response indicators before anesthesia induction,after tracheal intubation,and immediately after extubation,the levels of reproductive hormone 1 and 7 days after surgery,and perioperative adverse reactions were compared among the three groups.Results Compared with 4 hours after surgery,1 day after surgery,the bispectral index(BIS)of the three groups decreased,but compared with the group A and the group B,the group C was higher,compared with the group A,the group B was higher.The scores of visual analogue scale(VAS)and mini-mental state examination(MMSE)in the three groups all increased,but compared with the group A and the group B,the group C was lower,and compared with the group A,the group B were lower(P<0.05).After tracheal intubation and immediately after extubation,the levels of serum cortisol(COR),epinephrine(E)and Norepinephrine(NE)in the three groups were higher than those before anesthesia induction,and immediately after extubation were lower than those after tracheal intubation.After tracheal intubation and immediately after extubation,the group B were lower than those in the group A(P<0.05).Compared with 1 day after and before surgery,the levels of serum luteinizing hormone(LH),follicle stimulating hormone(FSH),estrogen(E2)and testosterone in the three groups 7 days after surgery all increased,and 1 day after surgery were higher than before,and 1 day after surgery,compared with the group A and the group B,the levels of serum FSH,T,E2 in the group C was higher,and compared with the group A,the group B was higher(P<0.05).The incidence of adverse reactions in the group C was 19.61%,both lower than 56.86%and 37.25%in the group A and the group B,and compared with the group A,the group B was lower(P<0.05).Conclusion Dexmetomidine has a good sedative and analgesic effect in benign prostatic hyperplasia surgery,which could effectively alleviate cognitive dysfunction of patients,reduce stress reactions,improve reproductive hormone levels,and have high safety.Among them,high-dose Dexmetomidine has a better application effect.
