Risk Factors of Coronary Atherosclerosis in Patients with Dilated Cardiomyopathy

Abstract：Objective To explore the risk factors of coronary atherosclerosis in patients with dilated cardiomyopathy(DCM)based on logistic regression model.Methods A total of 168 patients with DCM in Zhengzhou People's Hospital from April 2022 to April 2024 were selected as the research objects,and they were divided into an occurrence group(58 cases)and a non occurrence group(110 cases)according to the occurrence of coronary atherosclerosis.The clinical data of all patients were collected by hospital self-made questionnaire,and the influencing factors of coronary atherosclerosis in patients with DCM were analyzed.Results There were significant differences in the levels of diabetes mellitus,total cholerterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),lipoprotein-associated phospholipase A2(Lp-PLA2),macrophage inflammatory protein 1α(MIP-1α)and platelet membrane glycoprotein Ⅰb(GP Ⅰb)between the two groups(P＜0.05).Diabetes mellitus,neutrophil to lymphocyte ratio(NLR),monocyte to high-density lipoprtein cholesterol ratio(MHR),Lp-PLA2,MIP-1α and GP Ⅰb were the influencing factors of coronary atherosclerosis in patients with DCM(P＜0.05,OR=7.436,5.573,4.226,7.425,11.095,10.655).The logistic regression model was used to statistically analyze the data,and the predictive probability P of coronary atherosclerosis in patients with DCM was obtained.The receiver operating characteristic(ROC)curve was drawn according to the predictive value and the real value.The area under the curve(AUC)was 0.917(95％CI:0.876-0.962).When the log(P)＞2.85,the predictive sensitivity was 82.76％,and the specificity was 92.73％.Conclusion Diabetes mellitus,NLR,MHR,Lp-PLA2,MIP-1αand GP Ⅰb are important influencing factors of coronary atherosclerosis in patients with DCM,which can provide reference for early clinical evaluation of coronary atherosclerosis risk in patients with DCM and the development of corresponding intervention measures,so as to improve the prognosis of patients.
