Effect of Heparin Pretreatment Perfusion Device on ACT Value of Patients Undergoing Extracorporeal Circulation

Abstract:Objective To compare the effects of heparin pretreatment strategy on activated clotting time(ACT)value during extracorporeal circulation(ECC).Methods A total of 42 patients with infective endocarditis during ECC cardiac surgery in Fuwai Central China Cardiovascular Hospital from November 2020 to November 2021 were analyzed retrospectively.According to whether heparin pretreatment was performed on the perfusion device during operation,they were divided into two groups:heparin pretreatment group(20 cases)and non heparin pretreatment group(22 cases).The age,sex,body mass,ECC time,aortic occlusion time,use time of perfusion device,ACT value before,1,2 hours after use of perfusion device,the amount of heparin used in ECC,the amount of heparin added after use of perfusion device,and coagulation function,hematological examination,platelet count and complement activity on 2 hours after use of perfusion device,etc were collected for analysis and comparison.Results After the use of hemoperfusion devices in both groups of patients,the ACT values would shorten,and the difference was statistically significant(P<0.05).There was no significant difference in the ACT values before,1,2 hours after the use of the perfusion device,the amount of heparin added in ECC,and coagulation function,hematological examination,platelet count and complement activity on 2 hours after use of perfusion device.Conclusion After heparin pretreatment in ECC of patients with infective endocarditis,the ACT value in ECC could not be significantly affected,and the use of heparin in ECC could not be significantly reduced.
