Correlation Between Non-invasive Cardiac Output Measurement and Echocardiography Results in Patients with Septic Shock

Abstract:Objective To explore the correlation between non-invasive cardiac output measurement(NICOM)and echocardiography results in patients with septic shock.Methods Adult septic shock patients who require hemodynamic monitoring and were hospitalized in the Respiratory and Critical Care Medicine Department of Henan Provincial People's Hospital from November 2023 to February 2024 were selected as the research subjects.NICOM and echocardiography were used to measure the cardiac index(CI),stroke volume(SV),and left ventricular ejection fraction(EF)of the same patient.The consistency of results between two measurement methods were compared.Results A total of 95 patients were included,and the CI measured by NICOM and echocardiography were 3.5(2.4,4.0)L·min-1·m-2 and 3.2(2.5,3.9)L·min-1·m-2,there was no significant difference in measuring CI between the two methods(Z=0.32,P=0.975).The Bland-Altman analysis suggests that the average bias between the two methods was-0.02 L·min-1·m-2.The SV measured by NICOM and echocardiography were(55.46±13.33)mL and(52.72±13.11)mL,respectively.The correlation coefficient between the two methods was 0.71(P<0.05),and the Bland-Altman analysis suggests that the average bias between the two methods was 2.7 mL.The EF values measured by NICOM and echocardiography were(55.41±7.41)%and(54.58±6.34)%,respectively.The correlation coefficient between the two methods was 0.55.The Bland-Altman analysis suggests that the average bias between the two methods was 0.8%.The correlation coefficients of CI and systemic vascular resistance index(SVRI)were 0.917 and 0.891 using pulse indicator continuous cardiac output(PICCO)and NICOM methods(P<0.05),respectively.Conclusion The CI,SV and EF values measured by NICOM and echocardiography in adult septic shock patients are correlated and consistent,and can quickly and accurately obtain patient related cardiac indicators.It has broad application prospects in the treatment of critically ill patients.
