Abstract:Objective To compare the anesthesia effect of nalbuphine combined with ciprofol or propofol in painless gastrointestinal endoscopy in patients with coronary heart disease. Methods A total of 80 patients with coronary heart disease who underwent painless gastroenteroscopy in Fuwai Central China Cardiovascular Hospital from February 2021 to February 2023 were selected as the study objects, and the computer random program was divided into nalbuphine hydrochloride combined ciprofol group(H group)and nalbuphine hydrochloride combined propofol group(B group),with 40 cases in each group. The hemodynamic indexes[heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP)and pulse oximetry(SpO2)],gastroscopy indexes[induction time, operation time, awakening time, orientation recovery time],adverse reactions and simple mental status examination scale(MMSE)were compared between the two groups at 5 minues(T0)before anesthesia, after the eyelash reflex disappeared(T1),the time of admission(T2),the time of retreat(T3),and the time of awakening(T4).Results There were significant differences between group H and group B in terms of heart rate, systolic blood pressure and diastolic blood pressure(P<0.05).There were no significant differences in heart rate, systolic blood pressure, diastolic blood pressure and oxygen saturation between the two groups at different times(P>0.05).There were interaction effects between time points and groups of the above four indicators(P<0.001).The heart rate, systolic blood pressure and diastolic blood pressure of both groups were lower than those at T2 and T3(P<0.05),and the corresponding indexes in group B were lower than those in group H at the same time point(P<0.001).The induction time and directional recovery time of group H were shorter than those of group B,and there were statistical differences between the two groups(P<0.05),and the incidence of delayed awakening in group H was lower than that in group B(P<0.05).There was no significant difference in the occurrence of adverse reactions and MMSE scores before and after examination between the two groups(P>0.05).Conclusion The use of nalbuphine combined with ciprofol in painless gastrointestinal endoscopy in patients with coronary heart disease is more conducive to hemodynamic stability and lower incidence of adverse than nalbuphine combined propofol.
