Predictive Effect of Continuous Electronic Fetal Heart Monitoring of Abnormal Graphic Deceleration Duration Combined with Deceleration Area on Neonatal Acidemia

Abstract:Objective To investigate the predictive effect of continuous electronic fetal heart monitoring with abnormal graphic deceleration duration and deceleration area on neonatal acidemia.Methods Clinical data of 120 newborns diagnosed with acidemia in Luoyang Maternal and Child Health Hospital from January 2020 to January 2023 and 120 healthy newborns during the same period were retrospectively collected.Women with neonatal acidemia were divided into groups according to whether they had acidemia or not.The women with neonatal acidemia were the study group(120 cases),and the women with healthy newborns were the control group(120 cases).Clinical data such as maternal age,gestational age,body mass index,newborn weight,birth time,newborn sex,educational level,continuous electronic fetal heart monitoring abnormal graphical deceleration duration and 30 min deceleration area were collected.The influence factors of neonatal acidemia were analyzed by binary logistic regression,and the area under receiver operating characteristic was used to evaluate the predictive efficacy of influencing factors on neonatal acidemia.Results There were no significant differences in age,gestational age,body mass index,newborn weight,birth time,newborn sex and educational level between the two groups(P>0.05).The abnormal graph deceleration duration and deceleration area of the study group were higher than those of the control group,the difference was significant(P<0.05).Abnormal graph deceleration duration and deceleration area were independent risk factors for neonatal acidemia.The 30 min deceleration area,the abnormal pattern deceleration duration and the combined AUC area were 0.764,0.890 and 0.925,respectively.The combined AUC area was the largest,and the prediction efficiency was better.Conclusion The duration of abnormal graphic deceleration and the area of deceleration area of continuous electronic fetal heart monitoring are risk factors for neonatal acidemia,and the combination of the two can predict neonatal acidemia well.
