Relationship Between HIF-1α Gene Polymorphism and Susceptibility to Acute Cerebral Infarction and Poor Prognosis After Thrombolytic Therapy

Abstract:Objective To analyze the polymorphism of hypoxia inducible factor-1α(HIF-1α)gene in acute cerebral infarction,and to explore its impact on susceptibility to acute cerebral infarction and poor prognosis after thrombolysis.Methods A total of 282 patients with acute cerebral infarction admitted to Xuchang Hospital of Traditional Chinese Medicine from October 2020 to May 2023 were selected as the disease group,and 240 healthy people who underwent physical examination in Xuchang Hospital of Traditional Chinese Medicine during the same period were selected as the control group.The polymorphism of HIF-1α gene in disease group and control group was detected,and the genotype distribution and allele frequency of the two groups were compared.After 3 months of thrombolytic therapy in the disease group,the prognosis recovery was evaluated,and the patients were divided into a poor prognosis group(score>2,72 cases)and a good prognosis group(score ≤ 2,210 cases)based on the modified Rankin scale score.The HIF-1α gene polymorphism and clinical data were compared between different prognosis groups,and the factors related to poor prognosis were analyzed.Results The rs1 1549467 locus of HIF-1α gene in the disease group and control group contained two alleles(G and A)and three genotypes(GG,GA,and AA),which met the Hardy-Weinberg genetic balance test(P>0.05).Compared with the control group,the disease group had a higher frequency of GG genotype and G allele(P<0.05),and a lower frequency of GA genotype(P<0.05).In the comparison of HIF-1α gene polymorphism and clinical data between different prognosis groups,the GG genotype and G allele frequency at rs1 1549467 locus,the proportion of patients aged ≥ 65 years old,the proportion of large atherosclerotic cerebral infarction,the proportion of stroke involving zang-fu organs,the level of C-reactive protein(CRP)and the score of National Institutes of Health stroke scale(NIHSS)in the poor prognosis group were higher(P<0.05),the frequency of GA genotype was lower(P<0.05),and the time from onset to thrombolysis was longer(P<0.05).The risk factors for poor prognosis of patients in the disease group included age≥65 years old,large atherogenic cerebral infarction,stroke involving zang-fu organs,long time from onset to thrombolysis,high CRP level,high NIHSS score,GG genotype and G allele at rs1 1549467 of HIF-1α gene(P<0.05).Conclusion HIF-1α(rs11549467)gene polymorphism may be associated with the susceptibility to acute cerebral infarction,and the genotype and allele distribution of rs1 1549467 of HIF-1α gene in patients with different prognosis of acute cerebral infarction are different,and the GG genotype and G allele may be associated with poor prognosis of patients treated with thrombolytic therapy.
