Application Value of ApoB/ApoA1 Ratio in Elderly Cases with Chronic Obstructive Pulmonary Disease and Heart Failure

Abstract：Objective To investigate the application value of apolipoprotein B/apolipoprotein A1(ApoB/ApoA1)ratio in elderly cases with chronic obstructive pulmonary disease(COPD)and heart failure.Methods From April 2021 to April 2023,90 elderly patients with COPD in the 989th Hospital of the People's Liberation Army Joint Logistic Support Force were selected as the simple COPD group,and 88 elderly cases with COPD and heart failure were selected as the COPD and heart failure group.An automatic biochemical analyzer was used to determine the patient's serum Apo A1 and ApoB levels,and the ApoB/ApoA1 ratio was calculated.The related factors of elderly COPD patients with heart failure were evaluated by multivariate logistic regression analysis.Results The levels of ApoB/ApoA1,creatine kinase(CK),serum cardiac troponin Ⅰ(cTn Ⅰ),hypersensitive C-reactiveprotein(hs-CRP),systolic left anteroposterior diameter(LAD),right atrialup-down diameter(RAD)in the COPD combined with heart failure group were higher compared with COPD group alone,and the left ventricular ejection fractions(LVEF)levels were lower than in the COPD alone group(P＜0.05).With the gradual increase of cardiac function classification,the levels of ApoB/ApoA1 in elderly patients with COPD and heart failure gradually increased(P＜0.05).ApoB/ApoA1 was positively correlated with CK,cTn Ⅰ,hs-CRP,LAD,RAD,cardiac function classification,and negatively correlated with LVEF,the percentage of forced expiratory volume in one second(FEV1％pred),FEV1/FVC,diffusing capacity of the lung for carbon monoxide(DLCO)(P＜0.05).High levels of ApoB/ApoA1,hs-CRP and hyperlipidemia were related factors of COPD complicated with heart failure in the elderly(P＜0.05).The AUC of the ratio of ApoB/ApoA1 in predicting heart failure in elderly COPD was 0.882(95％CI:0.808-0.967).At the best cut-off value of 0.45,the sensitivity and specificity are 86.4％and 82.2％,respectively.Conclusion The ratio of ApoB/ApoA1 in elderly patients with COPD and heart failure is increased.ApoB/ApoA1 is related to elderly patients with COPD and heart failure.High level of ApoB/ApoA1 is related factors for elderly COPD and heart failure.The ratio of ApoB/ApoA1 may be used to diagnose elderly patients with COPD and heart failure with high sensitivity and specificity.
