Clinical Effect on Bone Setting Manipulation Combined with Percutaneous Curved Vertebroplasty for Osteoporotic Vertebral Compression Fracture Non-union

Abstract:Objective To investigate the clinical efficacy of osteoporotic vertebral compression fractures non-union(OVCF-NU)treated with percutaneous curved vertebroplasty(PCVP)compared with percutaneous kyphoplasty(PKP).Methods Forty patients with OVCF-NU who met the selection criteria from August 2020 to August 2021 were used as the study population and were divided into a test group(20 cases)and a control group(20 cases)according to the random number table method.Bone setting manipulation combined with PCVP was used in the test group and PKP was used in the control group.Information on treatment cost,operative time,intraoperative cement injection,intraoperative cement leakage,cement distribution,the height of the anterior border of the injured vertebrae,and Cobb's angle were collected and compared between the two groups.The visual analogue score(VAS)was used to evaluate pain and the Oswestry disability index(ODI)was used for functional evaluation.Results The operation and follow-up were completed successfully in both groups.The treatment cost of experimental group was lower than that of control group,and the operation time was longer than that of control group(P<0.05).There was no significant difference in bone cement injection amount between the two groups(P>0.05).VAS and ODI scores of two groups before surgery,1 day after surgery and 4 weeks after surgery showed a gradually decreasing trend(P<0.05).There were 4 cases of bone cement leakage in the experimental group and 3 cases in the control group(P>0.05).Compared with pre-operation,the height of anterior vertebral body margin increased and the Cobb angle decreased in both groups(P<0.05).There was no significant difference in the height of anterior vertebral margin and Cobb angle between the two groups before and after surgery(P>0.05).The difference of bone cement distribution between the two groups was statistically significant(Z=3.175,P=0.001).Conclusion Compared with PKP,Bone setting manipulation combined with PCVP in the treatment of patients with OVCF-NU not only achieves similar results but also has the advantages of less treatment costs and better cement distribution,which is worth applying in clinical practice.
