Clinical Characteristics of Children Infected with Omicron Variant with Febrile Convulsion

Abstract:Objective To investigate the clinical characteristics of children with Omicron variant infection combined with febrile seizures(FS).Methods The clinical data of 91 children hospitalized with Omicron variant infection with FS admitted to Children's Hospital of Zhengzhou University from December 13,2022 to January 9,2023 were also retrospectively analyzed.The children were divided into simple febrile convulsions(SFS)group(60 cases)and complex febrile seizures(CFS)group(31 cases)according to the type of FS,and the clinical characteristics and laboratory findings of the children in the two groups were compared.Results FS symptoms were mainly manifested as bilateral upward turning of the eyes in 55 cases(60.44%),gaze/strabismus in 31 cases(34.07%),and tight closure of the eyes in 5 cases(5.49%).Loss of consciousness was observed in 75 cases(82.42%).Cyanosis around the mouth was observed in 62 cases(68.13%).Twitching of limbs was observed in 56 cases(61.54%),clenching of both fists in 10 cases(10.99%),and weakness of lower limbs in 2 cases(2.20%).The teeth were closed in 37 cases(40.66%),foaming at the mouth in 11 cases(12.09%),salivation around the mouth in 8 cases(8.79%),vomiting(non-jetting)in 6 cases(6.59%),and distorted corners of the mouth in 2 cases(2.20%).There were 2 cases of falling(2.20%),2 cases of screaming(2.20%),and 1 case of urinary incontinence(1.10%).Respiratory and cardiac arrest in 1 case(1.10%);among 91 children,59 cases(64.84%)were male,32 cases(35.16%)were female,age 1.58(2.33,3.58)years,40 cases(58.83%)had a history of febrile convulsions(23 cases with unknown history of febrile convulsions),68 cases(74.73%)were vaccinated,52 cases in children aged 1 to 3 years(57.14%),62 cases(68.13%)had only one episode of febrile course,66 cases(72.53%)had one convulsive episode lasting less than 5 min.The differences in clinical typing and vaccination between the SFS and CFS groups were not statistically significant(P>0.05).Sixty-six cases(72.53%)had elevated interleukin-10(IL-10),37 cases(40.66%)had elevated interleukin-6(IL-6),10 cases(10.99%)had elevated interleukin-4(IL-4),2 cases(2.20%)had elevated interferon-γ(IFN-γ).The differences between the SFS and CFS groups in the levels of interleukin-2(IL-2),IL-4,IL-6,IL-10,tumor necrosis factor-α(TNF-α),IFN-γ,interleukin-17a(IL-17a),and interleukin-12P70(IL-12P70)and the percentage of elevation were not statistically(P>0.05);the SFS and CFS groups showed no statistically significant differences in the proportion of elevated white blood cells(WBC),neutrophils(NEU),lymphocytes(LYM),monocytes(MONO),platelets(PLT),C-reactive protein(CRP),calcitoninogen(PCT)elevated,sodium ions(Na+),chloride ions(Cl-),potassium ions(K+),calcium ions(Ca2+),lactate dehydrogenase(LDH),α-hydroxybutyrate dehydrogenase(HBDH),creatine kinase(CK),and creatine kinase isoenzyme(CK-MB)levels were not statistically significant(P>0.05).Conclusion Children with Omicron mutant infection combined with febrile convulsions are more male than female,and the age of onset is mainly 1 to 3 years old.IL-2,IL-4,IL-6,IL-10,TNF-α,IFN-γ,IL-17a,and IL-12P70 are not differentially expressed in children with simple febrile convulsions and complex febrile convulsions.
