Abstract:Objective To analyze the correlation between the transvaginal ultrasound blood flow parameters[systolic peak velocity(PSV),end diastolic velocity(EDV),resistance index(RI),peak systolic/diastolic velocity(S/D)]and serum levels of serine family A member 1(SERPINA1)and the effectiveness of conservative treatment in patients with uterine leiomyoma. Methods A total of 80 patients with uterine fibroids admitted to Henan Second Provincial People's Hospital from October 2019 to September 2023 were collected retrospectively as the study group, and another 80 healthy physical examination subjects as the control group. All patients with uterine fibroids were treated with conservative clinical treatment.According to the different clinical efficacy at 3 months of treatment, they were divided into effective and ineffective patients. The PSV,EDV,RI,S/D and serum SERPINA1 levels of the two groups at admission were compared, and the levels of each index at admission and 3 months of treatment in patients with different clinical efficacy were compare. The predictive value of combined detection of each index at 3 months of treatment for ineffective treatment were analyzed using receiver operating characteristic(ROC)curve and area under the curve(AUC),and the risk of ineffective treatment in patients with different levels were analyzed. Results The PSV,EDV and SERPINA1 of the study group were higher than those of the control group, while the expression of RI and S/D of the study group were lower than those of the control group(P<0.05).After 3 months of treatment,PSV,EDV and SERPINA1 in patients with ineffective treatment were higher than those with effective treatment, and the expressions of RI and S/D were lower than those with effective treatment(P<0.05).After 3 months of treatment, the AUC of PSV,EDV,RI,S/D and serum SERPINA1 levels were 0.763 and 0.836 respectively. After 3 months of treatment, patients with high PSV,EDV,SERPINA1 and low RI and S/D had a higher risk of treatment failure(P<0.05).Conclusion Blood flow parameters and serum SERPINA1 level of transvaginal ultrasound are related to the clinical efficacy of conservative treatment of uterine fibroids, and detection of its expression can effectively predict the therapeutic efficacy of conservative treatment of uterine fibroids, which has high clinical application value.
