Abstract:Objective The relationship between serum levels of miR-146a,cycliphilin A(CyPA),neuron specific enolase(NSE)and cognitive function in patients with epilepsy secondary to cerebral infarction was analyzed,in order to provide reference for clinical diagnosis and treatment of epilepsy secondary to cerebral infarction. Methods The clinical data of 200 patients with cerebral infarction in the Department of Neurology of the Third People's Hospital of Henan Province from April 2022 to December 2023 were retrospectively analyzed.According to whether cerebral infarction had secondary epilepsy, they were divided into the group without secondary epilepsy(158 cases)and the group with secondary epilepsy(42 cases).Serum levels of miR-146a,CyPA,NSE and cognitive function were compared between the two groups, and the relationship between serum levels of miR-146a,CyPA,NSE and cognitive function was analyzed,as well as the risk factors affecting secondary epilepsy after cerebral infarction. Results Compared with the group without secondary epilepsy, the serum miR-146a level was lower in the group with secondary epilepsy, and the levels of CyPA and NSE were higher(P<0.05).Compared with the non-secondary epilepsy group, the scores, language function, naming, visuospatial and executive ability, attention, memory, orientation and abstract thinking of Montreal cognitive assessment(MoCA)were lower in the secondary epilepsy group(P<0.05).The proportion of cortical infarct site, atrial fibrillation, presence of cardiogenic infarction and large area infarction in secondary epilepsy group were higher than those in non-secondary epilepsy group(P<0.05).According to the multi-factor data, low level of miR-146a,high level of CyPA,high level of NSE,location of cortical infarction, cardiogenic infarction, and large area infarction were the influencing factors of patients with epilepsy secondary to cerebral infarction(P<0.05).Conclusion Patients with epilepsy secondary to cerebral infarction will decrease the level of serum miR-146a,increase the levels of CyPA and NSE,and impair cognitive function. In addition, low level of miR-146a,high level of CyPA,high level of NSE,site of cortical infarction, cardiogenic infarction, and large area infarction were the influencing factors of patients with epilepsy secondary to cerebral infarction.Clinical patients with the above clinical characteristics can be closely paid attention to,and targeted measures can be carried out to reduce the risk of epilepsy secondary to cerebral infarction, reduce cognitive function impairment.
