Analysis of the Influencing Factors in Patients with Reflux Esophagitis

Abstract:Objective To explore the characteristics of esophageal motility in patients with gastroesophageal reflux disease(GERD)and the influencing factors in patients with reflux esophagitis(RE).Methods Retrospective analysis of case data of 184 GERD patients who received treatment for reflux related symptoms in the Department of Gastroenterology at the First Affiliated Hospital of Henan University of Science and Technology from November 2020 to November 2022,and underwent gastroscopy,high-resolution esophageal manometry,and 24-hour esophageal pH impedance monitoring.According to endoscopic results,the patients were divided into nonerosive reflux disease(NERD)group(98 cases)and RE group(86 cases).The demographic and lifestyle characteristics,esophageal motility,esophageal hiatus function,and 24-hour esophageal pH impedance monitoring between two groups were compared.The related factors were analyzed by univariate and multivariate logistic regression analysis.Results There were statistically significant differences in gender,weight,body mass index,alcohol consumption,proportion of combined hiatal flabby(HF)or hiatal hernia(HH),resting pressure of the lower esophageal sphincter(LES),LES length,percentage of ineffective swallowing,frequency of pH channel acid reflux,average acid clearance time,proportion of upright and lying down pH<4 time,proportion of all pH<4 time,longest reflux time,longer reflux times(>5 minutes),and impedance channel acid,mixed,and liquid reflux times between the two groups(P<0.05).There was no statistically significant difference in age,height,smoking history,LES relaxation rate,distal contractile integral(DCI),resting pressure of upper esophageal sphincter(UES),and weak acid reflux between the two groups(P>0.05).The results of multiple logistic regression showed that body mass index,alcohol consumption(<40 g of alcohol per day,4-7 days per week),percentage of ineffective swallowing,HF and HH were the influencing factors of RE incidence.Conclusion The influencing factors leading to RE are high body mass index,high frequency of alcohol consumption and a decrease in esophageal clearance and antireflux capacity at the esophagogastric junction.Abnormal antomy in esophagogastric junction should raise alerts to develop a reasonable and personalized treatment plan for GERD patients.
