Association of Embryonal Changes in Posterior Cerebral Artery with Severity of Acute Posterior Circulation Cerebral Infarction

Abstract:Objective To investigate the relationship between the changes of fetal type posterior cerebral artery(FTP)and the severity of acute posterior circulation cerebral infarction.Methods A total of 96 patients with acute posterior circulation cerebral infarction admitted to Nanyang Second People's Hospital from July 2021 to July 2022 were selected.According to the results of magnetic resonance angiography,the patients were divided into non-FTP group(56 cases)and FTP group(40 cases),in which 24 cases complete FTP(eFTP)and 16 cases partial FTP(pFTP).Clinical data,infarct distribution and posterior circulation vessel morphology were compared.The severity of infarction was assessed by the National Institutes of Health stroke scale(NIHSS).There were 23 cases of mild infarction,45 cases of moderate infarction and 28 cases of severe infarction.Then multivariate logistic regression was used to analyze the relationship between the posterior cerebral artery accompanying embryonic changes and the severity of acute posterior circulation cerebral infarction.Results There was no difference in clinical data between non-FTP group and FTP group(P>0.05).There were no differences between cFTP group and pFTP group in the anterior cerebral artery,middle cerebral artery,posterior cerebral artery and vertebrobasilar artery infarction,left vertebral artery stenosis,right vertebral artery stenosis,and basilar artery curvature and convex vascular morphology(P>0.05).The proportion of diabetes,pFTP and cFTP in moderate and severe infarction group was higher than that in mild infarction group(P<0.05).Multivariate logistic regression analysis showed that middle cerebral artery infarction,cFTP and pFTP were independent risk factors for the severity of acute posterior circulation cerebral infarction(OR>1,P<0.05).Conclusion The accompanying embryonic changes of the posterior cerebral artery and the distribution of infarction are closely related to the severity of acute posterior circulation cerebral infarction.Understanding the accompanying embryonic changes of the posterior cerebral artery and the distribution of infarction is of important reference value for the evaluation of cerebral infarction.
