Clinical Effects of Laparoscopic Common Bile Duct Exploration Treatment for Acute Biliary Pancreatitis at Different Time Points

Abstract:Objective To explore the clinical effects of laparoscopic common bile duct exploration(LCBDE)performed at different times in the treatment of acute biliary pancreatitis(ABP).Methods A retrospective selection of clinical data from 95 patients with ABP treated in the First Affiliated Hospital of Zhengzhou University from August 2021 to August 2023 was conducted.They were divided into group A(49 cases)and group B(46 cases)according to surgical timing.The former underwent LCBDE within 72 hours after diagnosis,while the latter underwent LCBDE within 72-120 hours after diagnosis.The surgical indicators of two groups were compared,as well as liver function[alkaline phosphatase(ALP),glutamyl transpeptidase(GGT),alanine aminotransferase(ALT)],biochemical indicators[diamine oxidase(DAO),serum amylase(AMS),intercellular cell adhesion molecule-1(ICAM-1)],immune function[immunoglobulin G(IgG),CD8+,immunoglobulin M(IgM)]before and after surgery between the two groups,and the complications of the two groups were also compared.Results There was no statistical difference in the amount of bleeding and the number of cases converted to open surgery between the two groups(P>0.05).Group A had shorter first exhaust time,first defecation time,hospital stay,surgery time and first time out of bed compared to group B(P<0.05).Compared with group B,group A had lower serum levels of ALP,GGT and ALT one week after surgery(P<0.05).Compared with preoperative levels,the serum levels of DAO,AMS and ICAM-1 in both groups showed a decreasing trend(P<0.05).Compared with preoperative,the IgG,CD8+and IgM indicators in both groups showed an upward trend(P<0.05).There was no statistically difference in the incidence of complications between the two groups(P>0.05).Conclusion Performing LCBDE within 72 hours after diagnosis in ABP patients can reduce immune function damage,promote rapid liver function recovery,improve biochemical indicators,shorten hospitalization time,and promote rapid recovery of patients.
