Construction and Evaluation of Nomogram Model of Malnutrition in Elderly Patients with Heart Failure

Abstract:Objective To investigate the influencing factors of malnutrition in elderly patients with heart failure,establish and evaluate a nomogram model,and provide a rapid method for health care workers to identify malnutrition in elderly patients with heart failure.Methods A total of 338 patients with heart failure admitted to Department of Cardiology of the First Affiliated Hospital of Zhengzhou University from January 2020 to December 2021 were selected as the study objects.NRS 2002 score was performed and laboratory test results of N-temrina pro-barin natriuretic peptide(NT-proBNP),albumin(ALB),hemoglobin,creatinine,uric acid,glomerular filtration rate(GFR),fasting blood glucose(FBG)were collected.The subjects were divided into malnutrition group and normal nutrition group according to ALB results.Then binary logistic regression was used to analyze the influencing factors of malnutrition.Finally,R software was used to construct nomogram model and calibration curve,and receiver operating characteristic(ROC)curve and calibration diagram were used to evaluate the accuracy of the model.Results Among the 338 subjects,the incidence of malnutrition was 21.6%(73 cases).The independent risk factors for malnutrition in elderly patients with heart failure were age>80 years old(OR=5.10,P=0.002),hospitalization days ≥ 8 days(OR=7.70,P<0.001),GFR<60 mL·min-1(OR=4.08,P=0.018),NT-proBNP ≥4 000 ng·L-1(OR=3.62,P=0.011),FBG>6.1 mmol·L-1(OR=7.44,P<0.001).A nomogram model for predicting malnutrition in elderly patients with heart failure was established,the area under the curve(AUC)value was 0.91,and the C-index was 0.808.The AUC for malnutrition by NRS 2002 score was 0.79.Conclusion The incidence of malnutrition in elderly patients with heart failure is high.Age,length of stay,GFR,NT-proBNP and FBG are all influencing factors of malnutrition.A nomogram model based on these influencing factors can effectively predict the risk of malnutrition in elderly patients with heart failure.
