Effect of Early Rehabilitation Treatment on Postoperative Hand Function in Patients with Hand Scar Contracture

Abstract:Objective To investigate the effect of early rehabilitation treatment on postoperative hand function in patients with hand scar contracture.Methods A retrospective non-randomized controlled study was used to study the clinical data of 60 patients with hand scar deformities admitted to the First Affiliated Hospital of Zhengzhou University from September 2019 to March 2022 in the Department of Burn Repair and Reconstructive Surgery.Scar contracture lasted for 3-12 months,with 32 males and 28 females aged from 6 to 30 years old.Patients were divided into early rehabilitation group(≤1 month)and late rehabilitation group(>1 month)according to whether the patients performed hand rehabilitation function exercise within 1 month after the incision.All patients received the same rehabilitation content,including scar skin care,topical anti-scar drugs,scars massage,active and passive joint activities,pressure therapy,orthosis wearing.The patients and their families were asked to come to the hospital for reexamination 3 and 6 months after the operation.The basic information and wound healing time of the two groups were compared.The Vancouver scar scale(VSS)was used to evaluate the hand scar hyperplasia of the patients before operation,3 and 6 months after operation.Jebsen hand function test was used to evaluate the changes in hand function of patients before treatment,3 and 6 months after surgery.The total active range of motion method was used to evaluate finger joint range of motion,and the excellent ratio was calculated.The incidence of postoperative adverse reactions in the two groups was recorded.Results The hand scar scores and the time required to complete the Jebsen hand function test of the two groups were not statistically significant(P>0.05).At 3 and 6 months after surgery,the hand scar scores in the early rehabilitation group were lower than those in the late rehabilitation group,and the time required to complete the Jebsen hand function test were shorter than those in the late rehabilitation group(P<0.05).The excellent ratio of the hand joint range of motion in the early rehabilitation group and late rehabilitation group before operation were 4/30 and 5/30,respectively,and the excellent ratio after 3 months of treatment were 28/30 and 22/30,respectively,the excellent ratio after 6 months of treatment were 29/30,23/30,respectively.The excellent ratio of mobility of hand joints in the early rehabilitation group was higher than that in the late rehabilitation group after 3 and 6 months of treatment(P<0.05).There was no skin necrosis,infection,blisters and other adverse reactions occurred in the two groups during the treatment.Conclusion Early rehabilitation treatment can improve hand scar hyperplasia,increase the range of motion of affected joints,and better restore hand function,thereby improving the quality of life of patients.
