Analgesic Effect of Ultrasound-guided Ropivacaine Continuous Intermuscular Sulcus Brachial Plexus Block Anesthesia on Different Types of Upper Limb Fractures

Abstract：Objective To investigate the analgesic effects of different types of upper limb fractures under the ultrasound-guided of ropivacaine continuous intermuscular sulcus brachial plexus block anesthesia.Methods A total of 35,37 and 36 patients with fracture of humerus,radius and ulna who underwent open reduction and internal fixation in Zhengdong Hospital,the First Affiliated Hospital of Zhengzhou University from October 2018 to November 2020 were selected respectively(groups A,B and C).The numerical rating scale(NRS)and interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)levels before and after the operation were compared in the three groups at different time periods after the operation,and the incidence of adverse reactions in the three groups was compared.Results The NRS scores of groups A and B were lower than those of group C at different time points after surgery(P＜0.05),but there was no statistical significance in the NRS scores of groups A and B at different time points after surgery(P＞0.05).The levels of IL-6 and TNF-αin all three groups were increased 48 hours after surgery,and the levels of IL-6 and TNF-αin groups A and B were lower than those in group C(P＜0.05),but there was no statistical significance between groups A and B(P＞0.05).The incidence of adverse reactions in groups A,B and C was 17.14%,18.91%and 16.66%,with no statistical significance(P＞0.05).Conclusion Ultrasound-guided brachial plexus block anesthesia can play a good role in postoperative analgesia for both humeral fracture and radius fracture patients.Patients with ulnar and radius fractures may need to combine ulnar nerve block to strengthen analgesia,so as to further inhibit inflammation and improve analgesia effect.
