Clinical Analysis of Calcitonin-Negative Medullary Thyroid Carcinoma

Abstract:Objective To analyze the characteristics of lymph node metastasis and surgical treatment of calcitonin(CT)-negative medullary thyroid carcinoma(MTC).Methods The clinical data of patients with sporadic or hereditary MTC who underwent surgery at Henan Cancer Hospital from January 2010 to July 2022 were analyzed retrospectively.According to the preoperative serum CT level,the patients were divided into CT negative group and CT positive group.The clinical data,immunohistochemical results and biochemical cure rate were compared between the CT negative group and the CT positive group.The relationship between clinical pathological factors and treatment outcome was analyzed in patients with negative CT scan.Results Of the 156 patients,144 were CT positive and 12 were CT negative.The rate of extensive surgery was higher in patients with positive CT than in those with negative CT.There were statistically differences in surgical extent,lymph node metastasis,and serum CEA levels between the two groups(P<0.05).The proportion of patients with Ki-67>10%in the CT positive group was higher than that in the CT negative group(P<0.001).The biochemical cure rate of the postoperative CT negative group was higher than that of the CT positive group(P<0.001).There was no statistically significant difference in biochemical cure outcomes among patients of different ages,tumor size,serum CEA levels,and surgical methods(P>0.05).Conclusion Patients with CT negative MTC have a lower likelihood of lymph node metastasis,and unilateral lobectomy can provide a better prognosis for CT negative MTC.Women are more likely to achieve biochemical cure in the CT negative group.
