Application of drug-induced sleep endoscopy in patients with obstructive sleep apnea hypnea syndrome

Abstract:Objective To analyze the application value of drug-induced sleep endoscopy(DISE) in patients with obstructive sleep apnea hypnea syndrome(OSAHS).Methods Fourty-seven patients with sleep-disordered breathing diagnosed by PSG was observed by endoscopy when they were induced to sleepiness by dexmedetomidine.DISE findings were characterized according to airway level, severity, and axis of collapse as well as the VOTE classification.These measurements were compared with PSG parameters.Results All subjects had velum-related obstruction.Twenty-nine subjects (62%) had airway obstruction related to the oropharyngeal lateral walls, 34 subjects (72%) had airway obstruction related to the tongue base, and 5 subjects (11%) had airway obstruction related to epiglottis.Only 3 subjects(6%) had single site obstruction, 9 subjects (19%) had two sites obstruction related to velum and oropharyngeal lateral walls, and 15 subjects (32%) had two sites obstruction related to velum and tongue base.There were 15 subjects (32%) had three sites obstruction related to velum oropharyngeal lateral walls and tongue base.The differences in BMI, AHI, ODI, average and the lowest SaO2 between subjects with and without oropharyngeal lateral walls-related obstruction were statistically significant (P<0.05).Conclusion The dynamitic change of velum, tongue base, oropharyngeal lateral walls and epiglottis of patients who was diagnosed as OSAHS can be seen clearly in the examination of endoscopy during drug-induced sleep.It can provide direct evidence to sleep surgery.
