Analysis of the factors affecting pathologic complete response of axillary lymph nodes after neoadjuvant chemotherapy

Abstract:Objective To investigate the factors affecting pathologic complete response of axillary lymph nodes after neoadjuvant chemotherapy.Methods Two hundred and twenty-seven breast cancer patients were collected from June of 2012 to February of 2016 in the Tumor Hospital affiliated to Zhengzhou University.Factors affecting the pCR of neoadjuvant chemotherapy were determinated by univariate analysis and multivariate Logistic regression analysis.Results Tumor size, shrinkage pattern, chemotherapy cycles, chemotherapy regimens, staging of lymph nodes, HER2 status on neoadjuvant chemotherapy for breast cancer were correlated to the pCR of axillary lymph nodes (P<0.05).Patients' age, menstrual status, ER and PR status, Ki-67 index and molecular subtypes were not correlated to the pCR of axillary lymph nodes (P>0.05).Multivariate analysis showed that tumor size, chemotherapy cycles, chemotherapy regimens, staging of lymph node were independent factors for the pCR of axillary lymph nodes.The larger the tumor size, the higher the staging of lymph nodes, the lower the pCR rate.Conclusion The regimens of anthracycline plus taxane can achieve a higher pCR rate.Tumor size≤5 cm, lymph node staging cN1, chemotherapy cycles≥6 can achieve a higher pCR.Trastuzumab can increase the pCR rate of the HER2 positive patients.
