Clinical efficacy of surgery through transsylvian -transinsular approach in the treatment of basal ganglia hematoma

Abstract:Objective To compare clinical efficacy of surgery through transsylvian -transinsular approach and transcortical -transtemporal approach in the treatment of primary intracerebral hemorrhage(PICH)of basal ganglia.Methods 1 65 patients with PICH of basal ganglia treated in the People’s Hospital of Wugong County from January of 2009 to April of 201 5 were select-ed and divided into observation group (transsylvian -transinsular approach,84 cases)and control group (transcortical -tran-stemporal approach,81 cases).The removal volumes of intracranial hematoma and prognosis were compared between two groups. Results The postoperative removal rate of hemorrhage in large area in observation group was 61 .90%,higher than control group (41 .98%),and the difference was statistically significant(P <0.05).The rates of rebleeding in observation group and control group were 3.57% and 7.41 % respectively,and the difference was statistically significant(P <0.05).The rates of good prog-nosis were 70.24% and 41 .98% respectively(P <0.05).Conclusion Surgery through transsylvian -transinsular approach is an effective treatment for PICH of basal ganglia,and it is characterized by higher hematomas removal rate,lower rate of rebleed-ing and good recovery of neivous function.
