Efficacy of arthroscopic synovectomy combined with radiation therapy for diffuse pigmented villonodular synovitis of the knee

Abstract：Objective To explore the efficacy of arthroscopic synovectomy combined with external -beam radiation therapy for diffuse pigmented villonodular synovitis(PVNS)of the knee.Methods A total of 26 PVNS patients that had been treated with arthroscopic synovectomy combined with external -beam radiation therapy from July of 2010 to December of 2012.The pa-tients were evaluated for pain swelling effusion range of motion and function of the whole leg.The postoperative short -term cura-tive effect was evaluated by Lysholm and IKDC.Recurrence was evaluated by MRI.Results All 26 patients were successfully followed up and were diagnosed as diffuse PVNS of the knee by pathological examination.Medial meniscus was involved in 2 ca-ses and ACL in 2 cases.There was slight pain in 1 case and swelling in 2 cases,but all patients did not suffer effusion.The Ly-sholm scores ascended from (60.96 ±1.52)to (86.31 ±1.17),the IKDC scores ascended from (61.27 ±1.60)to (86.92 ± 0.93).At the last follow -up,one of the patients recurred at the medial meniscus found by MRI,but the motion of the knee was well with no pain and swelling.The other patients did not suffer recurrence.Conclusion Arthroscopic synovectomy combined with RT might further reduce recurrence rate of diffuse PVNS of knee and assure the good surgical results with small surgical trau-ma,a rapid recovery and less cost of surgery.The knee activity and overall functional was satisfied.
