Application efficacy of community strengthened management base on PRECEDE-PROCEED model in patients with type 2 diabetes

Abstract：Objective To explore the application efficacy of community strengthened management base on PRECEKE-PRO-CEEK model in patients with type 2 diabetes. Methods 206 patients with type 2 diabetes were randomly divided into the inter-vention group(n=103)and control group(n=103). Intervention group was conducted by community strengthened management base on PRECEKE-PROCEEK model. The control group was conducted by routine community health management. The fasting blood glucose,2 h postprandial blood glucose,cholesterol,glycosylated hemoglobin,BMI and other indicators were compared. Results There was no significant difference in various indexes between two groups at baseline(P﹥0. 05). The systolic pres-sure,the fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin,low density lipoprotein in intervention group were superior to control group at final stage(P﹤0. 05). Conclusion The community strengthened management base on PRECEKE-PROCEEK model could have a better control of blood glucose levels in patients with type 2 diabetes and deserve the clinical expansion.
