The distribution and significance of glomerular IgG subclasses in children with Henoch -Schrnlein purpura nephritis

Abstract:Objective To investigate the distribution and significance of glomerular IgG subclasses in children with Henoch -Schrnlein purpura nephritis(HSPN).Methods Eighty -eight children with HSPN in the First Affiliated Hospital of Zhengzhou University from December of 2012 to May of 2014 were divided into IgG (+)group (group A)and IgG (-)group(group B) by the detection of IgG subclass.Clinical manifestations and pathological features were compared between two groups.Results The patients were divided into two groups according to renal pathology,group A(26 cases)accounting for 30%;group B (62 ca-ses)accounting for 70%.The age of onset,gender,injury time of kidney,24 -hour urinary protein quantity and renal pathological grades of the two groups had no significant difference (P >0.05).The infection and joint involvement of group A were higher than group B,and the difference was statistically significant(P <0.05).The immunofluorescence staining showed that IgG1 ten-ded to be highly expressed and its intensity was higher than other subclasses in HSPN.The positive rate and intensity of IgG4 were lower than other subclasses.Conclusion The HSPN children with IgG(+)were often accompanied by symptoms of joint and infection besides typical purpura.IgG1 deposition was priority to IgG2 and IgG3,and IgG4 deposition was least by immuno-fluorescence staining.IgG1 may play a dominant role in renal damage mediated by IgG immune complexes.
