The effect of rescue PTCA on the clinical prognosis of patien ts with acute myocardial infarction
Abstract:Objective:To observe the safety and clinical effica cy of rescue percutaneous transluminal coronary angioplasty (PTCA) of patients with acute myocardial infarction who had failed thrombolytic therapy or was not feasible to thrombolyze.Methods:9 cases of patients (age 63.2±6.6 years) who had definite acute myocardial infarction transferred directly to cath lab from emergent department.Infarction-related vessels were revascularized with balloon PTCA or stenting. IABP was inserted if necessary.Results:9 patients had 9 infarction-related vessels;7 patient s had TIMI flow grade 0,2 cases grade I;The time from severe chest pain to femor al artery puncture was 4.1±0.6 hour,procedural time was 51.9±11.3 minutes;TIMI flow restored to grade Ⅲ in all patients without any major complications (No- reflow phenomenon,acute occlusion,death,coronary bypass grafting,target vessel re-PTCA).Average following period was 7.6±3.9 months;EF was 63%±5%;The movement of infarction-related myocardial wall was completely normal in 7 of 9 patients and 2 patients only had slight hypokinesis;Q waves on electrocardialgram was decreased (3/9) or disappeared (6/9),Free-symptom living was 8 patients,only one patient had some tightness on his chest when he did moderate exercis.Con clusion: Rescue PTCA is a safe and effective treating measurement for th e patients who has acute myocardial infarction and has good mid and long-term p rognosis.
