Study on Changes in Serum sCD40L and sCD163 Levels and Their Relationship with Coronary Artery Lesions in Children with Kawasaki Disease Combined with Mycoplasma Pneumoniae Infection

Abstract:Objective To analyze the changes in serum soluble cluster of differentiation 40 ligand(sCD40L)and soluble cluster of differentiation 163(sCD163)levels and their relationship with the development of coronary artery injury(CAI)in children with Kawasaki disease(KD)combined with mycoplasma pneumoniae(MP)infection.Methods A to-tal of 114 children diagnosed with KD complicated with MP infection(KD complicated with MP group)and 114 children with MP infection alone(control group)between January 2020 to December 2024 were enrolled;based on the presence or absence of CAI,the pediatric patients with KD complicated with MP infection were further divided into CAI group(n=34)and non-CAI group(n=80).Multivariate unconditional Logistic regression was used to analyze the association between serum sCD40L and sCD163 levels and CAI in KD children complicated with MP infection;receiver operating characteristic(ROC)curve analysis was performed to assess the predictive value of serum sCD40L and sCD163 levels for CAI in pediatric patients with KD complicated with MP infection.Results Compared with the control group,pediatric patients in KD complicated with MP group had significantly higher levels of serum sCD40L and sCD163(P<0.05),the incidence of CAI in pediatric patients with KD complicated with MP infection was 29.82%(34/1 14),compared with non-CAI group,serum sCD40L and sCD163 levels of pediatric patients were significantly higher in the CAI group(P<0.05).Intravenous immunoglobulin(IVIG)non-respon-siveness,elevated creatine kinase isoenzyme MB(CK-MB),increased N-terminal pro-B-type natriuretic peptide(NT-proBNP),elevated sCD40L and elevated sCD163 were identified as independent risk factors for CAI in pediatric pa-tients with KD complicated with MP infection(P<0.05).The area under the curve(AUC)of serum sCD40L and sCD163 combined in predicting CAI in pediatric patients with KD and MP infection was 0.882,which was significantly larger than that of serum sCD40L and sCD163 alone(P<0.05).Conclusion The levels of serum sCD40L and sCD163 increased in pediatric patients with KD complicated with MP infection,sCD40L and sCD163 were independent risk factors for CAI,the combination of serum sCD40L and sCD163 has a high value in predicting CAI in pediatric patients with KD complicated with MP infection.
