Analysis of Risk Factors for Early Neurological Deterioration After Moderate Traumatic Brain Injury

Abstract:Objective To analyze the risk factors for early neurological deterioration in patients with moderate trau-matic brain injury(mTBI).Methods A retrospective analysis was conducted on the clinical data of 479 patients with mTBI admitted to the authors' hospital from January 1,2015 to December 31,2020.Based on whether the Glasgow coma scale(GCS)score decreased by≥2 within 24 h or emergency surgery was performed,patients were categorized into neu-rological deterioration group(n=45)and non-deterioration group(n=434).The baseline clinical characteristics of the patients between two groups were compared,and multivariate Logistic regression analysis was performed to identify inde-pendent risk factors for early neurological deterioration in patients with mTBI.Results Among the 479 patients with mTBI,45 experienced neurological deterioration,yielding an incidence rate of 9.39%.Univariate analysis identified oral aspirin use,GCS,injury severity score(ISS),fibrin degradation products,platelet count,D-dimer,aspartate amin-otransferase and midline shift as risk factors for rapid neurological deterioration,while multivariate Logistic regression analysis revealed that oral aspirin[odds ratio(OR)=2.882,95%confidence interval(CI):1.045-7.954],platelet count(OR=0.994,95%CI:0.988-1.000)and GCS(OR=0.727,95%CI:0.535-0.986)were independent risk fac-tors for early neurological deterioration in mTBI patients(P<0.05).Conclusion Patients with mTBI should be alert to the occurrence of early neurological deterioration.oral aspirin,platelet count and GCS are independent risk factors for early neurological deterioration of mTBI.
