Correlation Between Circulating Fibrocyte Levels and Infarct Size in STEMI Patients Treated with Primary Percutaneous Cor-onary Intervention

Abstract:Objective To analyze the relationship between the levels of circulating fibrocyte(CF)and the infarct size of patients after primary percutaneous coronary intervention(pPCI)for ST-segment elevation myocardial infarction(STEMI).Methods A prospective study was conducted to enroll 103 patients with acute STEMI who underwent pPCI from January 2020 to February 2024,the infarct size at 1 week and 6 months after pPCI were examined by cardiac mag-netic resonance imaging,and CF levels were measured 1 week after pPCI.Results STEMI patients were divided into low CF group(CF<5.46 × 105/ml,n=51)and high CF group(CF≥5.46 × 105/ml,n=52)with the median CF level of 5.46 × 105/ml as the critical value 1 week after pPCI.The incidence of major adverse cardiovascular events was higher in the high CF group(P<0.001).The baseline infarct size proportion was 25.67%(16.09%,32.17%)in the low CF group and 25.30%(22.70%,29.48%)in the high CF group,respectively,with no significant difference(P=0.537).However,the infarct size in the low CF group was significantly smaller than that in the high CF group 6 months after sur-gery[25.90%(17.45%,32.20%)vs.32.40%(27.35%,36.60%),P=0.001].Spearman rank correlation analysis showed that only CF level was positively correlated with infarct size 6 months after surgery(r=0.363,P<0.001).CF level predicted 0.838[95%confidence interval(CI):0.752-0.903]of subjects with infarct size ≥18%6 months after STEMI surgery.Conclusion Among STEMI patients treated with pPCI,postoperative CF levels are positively correlated with infarct size on cardiac magnetic resonance imaging at 6 months.CF levels can be used as a risk stratification tool or as a target for therapeutic response.
