Value of Metoprolol Tartrate Test in Electrocardiogram ST-T Changes in Diagnosis of Coronary Atherosclerotic Heart Disease in Young and Middle-aged Women

Abstract:Objective To investigate the clinical value of metoprolol tartrate test in the diagnosis of coronary ather-osclerotic heart disease[briefly coronary heart disease(CHD)]in young and middle-aged women with changes in electro-cardiogram ST-T.Methods The medical records of 104 female patients aged 18-59 with ST-T changes in electrocardio-graph and metoprolol tartrate test were retrospectively analyzed.The patients were divided into complete recovery group(positive result,n=71)and incomplete recovery group(negative and improved result,n=33)according to the ST-T re-covery results after medication.The clinical data of the two groups were collected,the resting states of the patients before medication were recorded and the electrocardiograph,heart rate and blood pressure of the patients were recorded at 1,1.5 and 2 h after oral administration of metoprolol tartrate 50 mg.The differences of general data,past medical history,fe-male-specific data and electrocardiograph characteristics,the proportion of confirmed CHD and drug side effects,and the differences of heart rate and blood pressure between the two groups were compared.Results The proportions of age,typical exertive symptom,history of hypertension,history of diabetes mellitus,history of hyperlipidemia and menopause in the incomplete recovery group were significantly higher than those in the complete recovery group(all P<0.05).Cor-onary angiography(CAG)was used as the gold standard for the diagnosis of CHD,and the proportion of patients diagnosed with CHD in the incomplete recovery group was significantly higher than that in the complete recovery group(P<0.05).There were 9 cases of drug side effects in the two groups,all of which were sinus bradycardia,and there was no significant difference between the two groups(P>0.05).The heart rates of patients between the two groups at 1,1.5 and 2 h after taking medicine were lower than those before;the heart rates of patients in complete recov-ery group at 2 h after taking medicine were higher than those at 1 and 1.5 h after taking medicine,and the heart rates of patients in incomplete recovery group at 2 h after taking medicine were higher than that at 1.5 h after taking medicine(all P<0.05).There was no significant difference in heart rates at each timing between the two groups(all P>0.05).Sys-tolic blood pressure(SBP)and diastolic blood pressure(DBP)of patients in 2 groups were lower than those before 1,1.5 and 2 h after taking medecine;the SBP of patients in the complete recovery group at 2 h after taking medicine were lower than those at 1 h after taking medicine,and the DBP of patients were lower than those at 1 and 1.5 h after taking medi-cine;SBP of patients in incomplete recovery group at 2 h after taking medicine were lower than those at 1 h after taking medicine(all P<0.05);SBP and DBP of patients in the incomplete recovery group were higher than those in the complete recovery group at all timing(all P<0.05).Conclusion Metoprolol tartrate test is one of the effective non-invasive screening tests for CHD,young middle-aged women have ST-T changes in electrocardiograph,and they have no typical symptoms and cardiovascular disease risk factors.If metoprolol tartrate test is positive,it is mostly functional ST-T chan-ges,which provides basis for clinical differential diagnosis and treatment,and has good clinical value.
