Study on Vision-Related Quality of Life in Patients After Artificial Keratoplasty

Abstract:Objective To evaluate the vision-related quality of life and its influencing factors in patients after artifi-cial keratoplasty.Methods A cross-sectional study was conducted to collect the demographic and clinical data of 127 pa-tients with bilateral corneal blindness who underwent monocular MICOF keratoprosthesis or Boston type Ⅰ keratopros-thesis implantation in author's hospital from January 2004 to December 2019,the Chinese national eye institute visual function questionnaire-25(CHI-NEI-VFQ-25)was used to assess vision-related quality of life.Pearson correlation analy-sis and Spearman correlation analysis were used to analyze the correlation between postoperative operated eye BCVA and vision-related quality of life.Multivariate stepwise linear regression was used to analyze the influencing factors of vision-related quality of life.Results Among the 127 enrolled patients,postoperative follow-up was(4.31±3.16)years,and the CHI-NEI-VFQ-25 composite score was(44.18±18.40).The scores of long distance activities,mental health,de-pendence,driving ability and comprehensive scores were significantly lower in patients whose time interval from surgery to questionnaire survey<2 years than those whose time interval from surgery to questionnaire survey≥2 years(all P<0.05);the scores of ocular pain,close up activities,role difficulty and total score were higher in patients with MICOF keratoprosthesis implantation than in patients with Boston type Ⅰ keratoprosthesis implantation(all P<0.05).The post-operative best corrected visual acuity(BCVA)and CHI-NEI-VFQ-25 composite score(r=-0.775,P<0.001),and ex-cept for ocular pain(r=0.139,P=0.120),other subscales were negatively correlated(all P<0.05).Postoperative operated eye BCVA and the time interval between surgery and questionnaire survey<2 years had negative predictive effect on the CHI-NEI-VFQ-25 com-posite score(β=-21.546,P<0.001;β=-5.698,P=0.029).Conclusion The vision-related quality of life of patients with bilateral corneal blindness improved after receiving artificial keratoplasty.The primary influencing factor for postoperative vision-related quality of life was the visual acuity of the operated eye after surgery,however,whether the time interval between artificial keratoplasty and the questionnaire survey less than 2 years also had an impact on vision-related quality of life.
