Current Situation of Fall Fear and Its Relationship with Frailty and Mental Resilience in Elderly Patients Undergoing Hip Re-placement

Abstract:Objective To investigate the current situation of fall fear and its relationship with frailty and mental re-silience in elderly patients with hip replacement.Methods A total of 286 elderly patients who underwent hip replacement surgery due to femoral neck fractures in the author's hospital from January 2021 to December 2023 were selected as the research subjects.General information of the patients was collected,the falls efficacy scale international(FES-I),Tilburg frailty indicator(TFI)and Connor-Davidson resilience scale(CD-RISC)were used to assess the patients' fear of falling,frailty and mental resilience respectively.The correlation among total scores of FES-I and TFI,CD-RISC in elderly pa-tients undergoing hip replacement surgery was explored by Pearson correlation analysis;the influencing factors of FES-I total score in elderly patients undergoing hip replacement surgery were analyzed by multiple linear regression analysis.Results The incidence of fear of falling in elderly patients undergoing hip replacement surgery was 56.99%(163/286),with FES-I total score of(35.91±4.21),TFI total score of(5.86±1.03)and CD-RISC total score of(54.18+7.15).Correlation analysis showed that the FES-I total score of elderly patients undergoing hip replacement surgery was positive-ly correlated with the TFI total score and negatively correlated with the CD-RISC total score(r=0.569,-0.449,all P<0.05).Female patients,those over 70 years old,those with high school education or lower,those whose main caregivers were nursing assistants,those with history of falls and those chronic diseases≥1 had higher FES-I total score(all P<0.001).Multiple linear stepwise regression analysis showed that age>70,who has history of falls,types of chronic diseases≥1 and elevated TFI total score were independent risk factors for the FES-I total score of elderly patients undergoing hip re-placement surgery,while the elevated CD-RISCtotal score was its protective factor(all P<0.001).Conclusion The inci-dence of fall fear in elderly patients with hip replacement is high,which is significantly correlated with frailty and mental resilience,and effective intervention measures can be taken to reduce the degree of frailty and improve the level of mental resilience,so as to reduce the occurrence of fall fear of patients.
