Interpretation of CYP2C19 Genetic Testing for Oral P2Y12 Inhibitor Therapy:A Scientific Statement from the American Heart Association

Abstract:In June 2024,the American Heart Association(AHA)published a scientific statement titled CYP2C19 Genetic Testing for Oral P2Y12 Inhibitor Therapy:A Scientific Statement from the American Heart Associ-ation in the journal Circulation(hereafter referred to as the Statement).The Statement primarily addresses the role of CYP2C19 genetic testing,a drug-metabolizing enzyme,in guiding the use of oral P2Y12 inhibitor antiplatelet therapy.By synthesizing the latest clinical trial evidence,the Statement clearly demonstrates that current data support the implemen-tation of CYP2C19 genotyping prior to prescribing oral P2Y12 inhibitors in patients with acute coronary syndromes(ACS)or those undergoing percutaneous coronary intervention(PCI).Based on genetic test results,carriers of the CYP2C19 loss-of-function genotype should be prescribed potent P2Y12 inhibitors such as ticagrelor or prasugrel,while non-carriers may safely use clopidogrel,a widely utilized antiplatelet agent.This genotype-guided strategy effectively re-duces ischemic events without increasing bleeding risks.The Statement also elaborates on fundamental principles for se-lecting oral P2Y12 inhibitors,providing valuable insights for clinical practice in China.
