Establishment and Application of Drug Use Evaluation Criteria for Betahistine Hydrochloride Injection Based on AHM-TOP-SIS Method

Abstract:Objective To establish the drug use evaluation(DUE)criteria of betahistine hydrochloride injection and pro-vide reference for improving its rational clinical use.Methods Based on the instruction of betahistine hydrochloride injection,clinical guidelines and expert consensus,the DUE criteria were established,the rationality of medication of 200 inpatients treated with betahistine hydrochloride injection in authors' hospital from June 2023 to May 2024 was evaluated according to the DUE cri-teria;the attribute hierarchy model(AHM)was used to assign weights to the index layer,and the technique for order preference by similarity to an ideal solution(TOPSIS)was used to calculate the closeness(Ci)of each medical record to the optimal solu-tion,so as to obtain the evaluation results of rational drug use.Results Among the 200 patients,27 cases(13.50%)had Ci≥0.8,150 cases(75.00%)had 0.6≤Ci<0.8,and 23 cases(11.50%)had C,<0.6.The irrational drug use was mainly mani-fested in inappropriate solvent dosage,failure to preferentially select or switch to oral preparations(unreasonable cost-effective-ness)when oral administration was possible,situations requiring cautious use(the precautions are not reasonable),inappropriate indications,etc.The 10 DUE criteria for betahistine hydrochloride injection were indications,contraindications,cost-effective-ness,usage and dosage,solvent selection,precautions,special populations,combination use,efficacy evaluation and adverse re-actions,with weight coefficient of 0.104,0.103,0.083,0.076,0.073,0.056,0.095,0.072,0.083 and 0.073 respectively.The optimal solution of 10 indexes was 10,and the worst solution was 0.Conclusion The DUE criteria for betahistine hydrochloride injection established based on the AHM-TOPSIS method can be used for the rationality evaluation of this drug.The evaluation results suggested that there are problems such as inappropriate solvent selection,cost-effectiveness,precautions,indications,etc.in the use of betahistine hydrochloride injection in the author's hospi-tal,and supervision needs to be strengthened.
