Clinical Research of Two-dimensional Shear Wave Elasto-graphy Combine with PALBI Score and APRI in Predicting Post-hepatectomy Liver Failure in Hepatocellular Carcinoma

Abstract：Objective To investigate the value of two-dimensional shear wave elasto-graphy(2D-SWE)parameters combine with platelet-albumin-bilirubin(PALBI)score and aspartate aminotransferase/platelet ratio index(APRI)in pre-dicting post-hepatectomy liver failure(PHLF)in hepatocellular carcinoma(HCC)patients.Methods A total of 140 pa-tients with primary HCC who underwent hepatectomy in author's hospital from March 2020 to March 2024 were selected as the study subjects.Preoperative 2D-SWE was performed to evaluate PALBI score and calculate APRI;the occurrence of PHLF was counted after operation,and the patients were divided into PHLF group(n=30)and non-PHLF group(n=110).Multivariate Logistic regression analysis was used to explore the factors of PHLF in HCC patients.The receiver operating characteristic(ROC)curve was used to analyze the value of 2D-SWE liver stiffness measurement(LSM),PAL-BI score and APRI in predicting PHLF in HCC.Results The scores of LSM and PALBI and APRI in PHLF group were higher than those in non-PHLF group(all P＜0.05).High LSM,APRI and PALBI scores were risk factors for PHLF in HCC patients(P＜0.05).The area under the curve(AUC)of LSM,PALBI scores and APRI predicting PHLF in HCC patients was 0.714,0.735 and 0.759 respectively,the AUC of the three combined prediction was 0.969,which was higher than that of any single index.Conclusion The increase of preoperative 2D-SWE parameters LSM,PALBI scores and APRI in HCC patients is related to the occurrence of PHLF after operation.The predictive value of LSM combined with PALBI score and APRI predicting PHLF in HCC patients is high.
