Impact of Electrode Sequence Adjustment on Pacing Voltage Threshold During Transesophageal Cardiac Electrophysiological Examination

Abstract:Objective To investigate the effects of esophageal electrode catheter sequence adjustment(including in-terelectrode spacing and electrode polarity)on pacing voltage threshold during transesophageal cardiac electrophysiological examination.Methods Forty-six patients with pacing voltage thresholds exceeding 15 V undergoing transesophageal car-diac electrophysiological examination at the authors' hospital were randomly selected.Pacing voltage thresholds were sys-tematically analyzed through adjustments of interelectrode spacing and electrode polarity.Results Standard electrode con-nections were maintained:the red alligator clip was connected to the positive stimulation pole,the black clip to the nega-tive stimulation pole,and white clips to the mapping electrodes.Voltage reduction of 2-16 V was achieved in 42 cases through optimized electrode sequence adjustment.The optimal electrode sequences were identified as follows:red-white-black-white(38.10%),black-white-red-white(30.95%),red-black-white-white(19.05%),black-red-white-white(4.76%),red-white-white-black(4.76%)and black-white-white-red(2.38%).Conclusion Pacing voltage threshold demonstrates significant correlation with interelectrode spacing,optimal thresholds are obtained when connecting stimula-tion electrodes to poles 1 and 3.Electrode polarity shows minimal influence on thresholds when spacing remains constant.This methodology enables 2-16 V pacing voltage reduction,substantially enhancing procedural comfort and promoting clinical adoption of transesophageal cardiac electrophysiological examination.
