Efficacy of Kirschner Wire Protractor Assisted Repositioning Combined with External Fixator in the Treatment of Salter-Har-ris Type Ⅱ Epiphyseal Injury of the Distal Tibia in Adolescents

Abstract:Objective To observe the clinical efficacy of Kirschner wire protractor assisted repositioning combined with external fixator in the treatment of Salter-Harris type Ⅱ epiphyseal injuries in the distal tibia,and the validity,feasi-bility and limitations of this technique were also assessed.Methods A total of 10 patients with Salter-Harris type Ⅱ epi-physeal injuries of the distal tibia admitted to author's hospital from December 2020 to June 2023 were treated with Kir-schner wire protractor assisted repositioning combined with external fixator.The preoperative hospitalization days,length of the incision,operative time,intraoperative blood loss,soft-tissue complications,time to fracture healing,fracture healing rate and the long-term complications after surgery(deformity healing,premature epiphyseal closure,limb short-ening,etc.)were recorded,and at the last follow-up,American society of orthopaedic foot and ankle surgery(AOFAS)ankle-hindfoot score was used to evaluate the recovery of ankle joint function.Results Ten patients were followed up for a period of(17.30±3.70)months,the preoperative hospitalization stays were(5.20±1.80)d,the incision length was(5.40±1.10)cm,the operative time was(80.00±21.50)min,the intraoperative blood loss was(31.00±17.90)ml.All incisions healed by first intention,and no soft tissue complications such as infection and necrosis occurred;the frac-ture healing time was(2.00±0.50)months;the fracture healing rate was 100%;no bone bridge formation,uneven ar-ticular surface and premature epiphyseal closure were found in postoperative X-ray;there was no shortening or rotation deformity of the affected limb during the follow-up;the average AOFAS ankle-hindfoot scores were(92.30±4.20)at the last follow-up.Conclusion For pa-tients with Salter-Harris type Ⅱ epiphyseal injury of the distal tibia,the kirschner wire protractor is conducive to intraoperative reduction,and the external fixator provides reliable fixation.This technique can reduce iatrogenic damage to the epiphysis,causes relatively less trauma,and represents an option for treating Salter-Harris type Ⅱ epiphyseal inju-ry of the distal tibia,with satisfactory short-term outcome.
