Analysis of Factors of Postoperative Recurrence in Children with Adenoid Hypertrophy Combined with Otitis Media with Ef-fusion and Construction of a Risk Prediction Nomogram Model

Abstract：Objective To explore the influencing factors of postoperative recurrence in children with adenoid hyper-trophy(AH)and otitis media with effusion(OME),and construct a risk prediction nomogram model.Methods A total of 476 children with AH combined with OME who treated from January 2019 to April 2023 were selected as the study subjects.The children were divided into a study group(n=333)and verification group(n=143)according to a ratio of 7:3.The child patients in the study group were subdivided into recurrence group(n=62)and non-recurrence group(n=271)based on the recurrence rate 1 year after surgery.Data of patients were collected.The influencing factors of postop-erative recurrence of children with AH combined with OME were analyzed by multivariate Logistic regression analysis.The risk prediction nomogram model for postoperative recurrence of patients with AH combined with OME was construc-ted by R software.The predictive performance of the risk prediction model was evaluated and constructed by receiver oper-ating characteristic(ROC)curve and the Hosmer-Lemeshow(H-L)fitting test.Results The numbers of children＜6 years old,allergic rhinitis,cleft palate,recurrent respiratory infections,mastoid dysplasia and degree of hypertrophy of adenoid Ⅲ-Ⅳ in the recurrence group were significantly higher than those in the non-recurrence group(all P＜0.05).The results of Logistic regression analysis showed that age,allergic rhinitis,cleft palate,recurrent respiratory infections,mastoid dysplasia and degree of hypertrophy of adenoid were the influencing factors for postoperative recurrence of AH combined with OME(P＜0.05).H-L fitting test showed that,the consistency of the risk prediction nomogram model was well(P＞0.05).Conclusion Age,allergic rhinitis,cleft palate,recurrent respiratory infections,mastoid dysplasia and degree of hypertrophy of adenoid are the influencing factors for postoperative recurrence of AH combined with OME.The risk prediction nomogram model based on this has good consistency and discrimination.
