Clinical Evaluation of Tranexamic Acid in Reducing Perioperative Blood Loss for Calcaneal Fractures

Abstract:Objective To evaluate the clinical efficacy of tranexamic acid (TXA) in reducing the perioperative blood loss for calcaneal fracture.Methods From January 2010 to December 2012, 90 patients with close calcaneal fractures in authors' hospital were randomly divided into TXA group (n=45) and control group (n=45).The TXA group and control group were respectively received intravenous administration of TXA (15 mg/kg) and 0.9% NaCl solutions before surgery.Open reduction and internal fixation were performed in all factures patients.The intraoperative and postoperative blood loss, 1-day routine blood test, adverse event incidence and wound complications were compared between two groups.Results All patients were followed up for 3 months.There were no significantly differences in baseline indexes, operation time, intraoperative blood loss, transfusion volume, postoperative 48-hour blood loss, platelet count, prothrombin time, activated partial thromboplastin time, vascular adverse event incidence between two groups (P>0.05).The first 24-hour postoperative blood loss of TXA group was less than that of control group, postoperative hemoglobin and hematocrit of TXA group were obviously higher than those of control group, and the incidence of wound complications of TXA group was under that of control group (all P<0.01).Conclusion Preoperative single-dose of intravenous TXA can effectively reduce postoperative blood loss and rate of wound complications in patients with calcaneal fracture, but can not increase the adverse event incidence.It has a good curative effect.
