Abstract:Objective To compare the effect of transvaginal lesions removal and laparoscopic lesions removal on perioperative indexes, negative time of blood human chorionic gonadotropin (HCG) and menstrual recovery time in patients with cesarean scar pregnancy (CSP) of gestational age≥10 weeks. Methods The clinical data of 300 CSP patients with gestational age≥10 weeks in authors' hospital were analyzed retrospectively.According to the operation mode, all patients were divided into the transvaginal group (n=158) and laparoscopic group (n=142).The intraoperative blood loss, operation time, hospital stay, hospitalization expenses, negative time of blood HCG and menstrual recovery time were compared between the two groups. The postoperative pain degree was evaluated with the visual analogue scale (VAS), and the incidence of perioperative complications between two groups was analyzed. Results There were no significant differences in the intraoperative blood loss, operative time, length of hospital stay between two groups (P>0.05).The hospitalization expenses of the transvaginal group was significantly less than that of the laparoscopic group (P<0.05).No significantly difference was found in the negative time of blood HCG or menstrual recovery time between two groups (P>0.05).The VAS score of the transvaginal group was significantly higher than that of the laparoscopic group (P<0.05).No complications were found in all groups. Conclusion Both of transvaginal lesions removal and laparoscopic lesions removal are effective in the treatment of CSP.The former has higher economic benefit, while the latter has less pain.Clinicians should choose the proper operation according to the patients' condition.
