Clinical Efficacy of Percutaneous Transforaminal Endoscopic Discectomy for Adjacent Segments Lumbar Disc Herniation or Stenosis After Lumbar Spinal Fusion

Abstract:Objective To analyze the curative effects of using percutaneous endoscopic lumbar discectomy (PELD) to treat adjacent segments lumbar disc herniation or stenosis after lumbar spinal fusion.Methods Thirty-one patients with disc herniation or stenosis after lumbar fusion were retrospectively analyzed and followed up from February 2008 to February 2016 in author's hospital.Visual analogue scale (VAS),Japanese Orthopaedic Association (JOA) scores and modified MacNab standard were used to assess the curative effects of patients after surgery and at 1 year.Results All operative time ranged from 45 to 83 minutes,averaged 62.5 minutes.Bleeding volume was from 5 to 20 ml,averaged 10 ml.Hospital stay was 3-14 d,averaged 6.5 d.All patients were followed up for 10-31 months,averaged 18.3 months.Preoperative VAS scores were (7.29 ± 0.61) points,VAS scores after operation 3 days were (2.23 ± 0.41)points,the last follow-up VAS scores were (1.23 ± 0.32) points.There were significantly differences between preoperative and postoperative scores (P<0.01).Preoperative JOA scores were (9.95 ± 0.62) points,the scores on the day of discharge were (18.17 ± 2.18) points,the last follow-up scores were (28.20 ± 2.08) points,and there were significantly differences between pre-operation and the day of discharge,post-operation (P<0.01).Based on the lumbar spine JOA score,24 cases were excellent,4 good,2 fair and 1 poor.The good rate was 90.3% at 1 year after operation.Conclusion PELD in treatment of lumbar intervertebral disc herniation or stenosis of the adjacent section has exact effects,with less trauma,shorter operative time,faster recovery,fewer postoperative complications.
