Exploration of Key Technique Demanding for Modern Combat Casualty Care Basing on Analysis of the Causes of Potentially Survivable Death

Abstract:Objective To explore the key demanded techniques for modern combat casualty care by analyzing the causes of potentially survivable death(PSD) in recent wars.Methods Publications related to combat casualty care were reviewed and analyzed for main causes of PSD for casualties killed in action and died of wounds,respectively.And then the key techniques needed for modern combat casualty care were explored based on these causes.Results The causes of PSD for casualties killed in action and died of wounds changed remarkably in recent wars.In modern wars such as Operation Enduring Freedom and Operation Iraqi Freedom,the leading causes of PSD for casualties killed in action were major bleeding (85 %),airway obstruction (8 %) and tension pneumothorax (1 %),while the leading causes of PSD for died of wounds were major bleeding (80%),severe traumatic brain injury (9 %) and multiple organ failure (8 %).Targeting these causes,attention should be paid to techniques such as controlling of major bleeding and management of airway obstruction,et al,during the first aid stage;while in level Ⅲ medical facility,attention should be paid to techniques such as major bleeding and resuscitation strategy,damage control strategy for severe traumatic brain injury,intensive monitoring and care for severely injured casualties.Furthermore,training could reduce mortality remarkably,and thus great importance should be attached to training for these targeted techniques.Conclusion In order to reduce mortality in modern war,the key demanded first aid techniques are controlling of major bleeding and management of airway obstructian,et al,while in level Ⅲ medical facility,include bleeding control and resuscitation strategy for major bleeding,damage control strategy for severe traumatic brain injury,et al.In addition,training for these techniques are important.
