Clinical Analysis of Pathogens and Resistance of Peritoneal Dialysis-Associated Peritonitis in Tropical Regions

Abstract:Objective To analyze pathogens and resistance of peritoneal dialysis-associated peritonitis (PDAP) in tropical regions of Hainan province.Methods The pathogens,drug-resistant bacteria and clinical outcomes of 127 patients with PDAP in the second affiliated hospital of Hainan Medical College from January 2012 to January 2016 were collected and analyzed retrospectively.Results A total of 124 episodes (63.59%) in 127 patients occurred 195 episodes of PDAP were pathogenic culture positive,in which gram-positive bacteria were found in 68 episodes (54.84%),Gram-negative bacteria in 36 episodes (29.03%),fungi in 10 episodes (8.06%),and mixed bacteria in 10 episodes (8.06%).Two kinds of pathogenic bacteria were found in 9 eposodes (7.26%),and 3 kinds in 1 eposode (0.81%).Gram-positive bacteria were susceptible to vancomycin and linezolid,and resistant to penicillin (88.24%),erythromycin (83.82%),cefazolin (66.18%),oxacillin (63.23%),successively.Gram-negative bacteria were susceptible to imipenem,piperacillin/ tazobactam and cefoperazone/ tazobactam (all 0),and resistant to cefazolin (80.56%),ampicillin (63.89%),ceftriaxone (58.82%),successively.Fifteen patients died,including fungal infection in 7 episodes,gram-negative in 4 episodes,gram-positive in 2 episodes and mixed infection in 2 episodes.Conclusion Gram-positive bacteria are the main pathogens of PDAP.In the initial treatment of peritonitis,antibiotic therapy is on the basis of pathogenic bacteria detection and their susceptibilities.Patients have poor prognosis with fungal peritonitis.
