Clinical Observation of PCV-VG on Respiratory Mechanics Used for Retroperitoneal Laparoscopic Surgery

Abstract:Objective To investigate the effect of pressure-controlled ventilation-volume guaranteed (PCV-VG) on respiratory mechanics under general anesthesia for retroperitoneal laparoscopic surgery.Methods Sixty patients with retroperitoneal laparoscopic surgery were randomly divided into PCV-VG group and volume controlled ventilation (VCV) group,with 30 cases in each group.Tracheal intubation was conducted after similar anesthesia drug induction and same respiratory parameters.The tidal volume (VT),Peak airway pressure (PPEAK),mean airway pressure (PMEAN) and end expiratory carbon dioxide partial pressure (PETCO2) were recorded at point of 5-minute after intubation (T0),30-,60-,90-and 120-minute after pneumoperitoneum (T1~T4).The airway resistance,pneumoperitoneum lung compliance (CD) and arterial blood-end expiratory carbon dioxide partial pressure (Pa-ETCO2) were compared,meanwhile,extubation time,postoperative hospitalization stays and pulmonary complications after surgery detected by X-ray were compared.Results PPEAK,PMEAN and Pa-ETCO2 values at T1,T2,T3 and T4 in PCV-VG group were lower than those in VCV group(P<0.05).CD values at T2,T3 and T4 in PCV-VG group were higher than those in VCV group (P<0.05).The hospital stays in PCV-VG group were shorter than those in VCV group (P<0.05).Conclusion PCV-VG mode undergoing laparoscopic surgery increases lung compliance,reduces airway resistance,dead space ventilation,exhaust CO2 and hospital stays.
