Study on the Impact of Hospital-Acquired Infections on Hospitalization Costs for Acute Leukemia Chemotherapy Patients Using Propensity Score Matching

Abstract:Objective:To explore the direct economic burden incurred by acute leukemia patients undergoing chemotherapy due to hospital-acquired infections.Methods:A total of 496 patients with acute leukemia undergoing chemotherapy admitted to the sample hospital between 2022 and 2023 were selected as study subjects.Among them,78 patients developed hospital-acquired infections(infection group),while 418 patients did not develop such infections(non-infection group).A propensity score matching(PSM)method was employed to achieve a 1:1 match between the two groups.Statistical analysis was conducted on the matched patients' length of hospital stay and hospitalization costs.Results:After PSM-balanced randomization,the median hospital stays for the matched infected and non-infected groups were 25 days and 10 days,respectively,and the total hospital costs were 31 434.89 yuan and 11 826.09 yuan,with statistically significant differences(P<0.05).The costs for consultation fees,Western medicine,treatment,examinations,Chinese herbal medicine,laboratory tests,blood transfusions,bed occupancy,nursing care,and medical supplies were all higher in the infection group than in the non-infection group,with all differences statistically significant(P<0.05).The greatest difference between the two groups was in Western medicine costs(11 383.78 yuan),while the highest ratio was observed in Chinese herbal medicine costs(3.79).Conclusion:Hospital-acquired infections in acute leukemia patients prolong hospitalization and increase medical costs;infection prevention and control measures for these patients should be strengthened.
