Comparative Analysis of Ultrasound Features and TSH,Tg,TgAb,and TPOAb Results Between Micro and Non-Micro Papillary Thyroid Carcinoma

Abstract:Objective:To analyze the ultrasound characteristics and serological differences among papillary thyroid carcinomas(PTC)of varying sizes,thereby providing a basis for the diagnosis and treatment of papillary thyroid carcinoma.Methods:A total of 101 patients who underwent thyroidectomy at the First Hospital of Lanzhou University between October 2022 and October 2024 and were pathologically diagnosed with papillary thyroid carcinoma were enrolled in this study.Based on the maximum tumor diameter,they were divided into Group A(micro-papillary thyroid carcinoma,maximum diameter≤1 cm)comprising 60 cases and Group B(maximum diameter>1 cm)comprising 41 cases.The borders,echogenicity,aspect ratio,calcifications,blood flow,capsular invasion,and lymph node metastasis of papillary thyroid carcinoma in two patient groups were analyzed and compared under ultrasound guidance.The results for thyroid-stimulating hormone(TSH),triglycerides(TG),antithyroglobulin antibodies(TGAb),and anti-thyroid peroxidase antibodies(TPOAb)were compared between the two groups.Results:Univariate analysis revealed statistically significant differences between the two groups in terms of margins,echogenicity,aspect ratio,calcification,blood flow,and lymph node metastasis(P<0.05).The differences in TSH,Tg,TgAb,and TPOAb levels between the two groups were not statistically significant(P>0.05).Multivariate analysis revealed that calcification,blood flow,and lymph node metastasis were associated with increased volume in papillary thyroid carcinoma.Conclusion:Micro-and non-micro-papillary thyroid carcinomas exhibit differences in calcification and blood flow,necessitating careful clinical differentiation.Non-micro-papillary thyroid carcinomas demonstrate enhanced lymph node metastasis potential compared to micro-papillary carcinomas,underscoring the importance of early prevention and treatment for papillary thyroid carcinoma.No differences were observed in serum markers between micro-and non-micro-papillary thyroid carcinomas.
