Correlation Analysis of Ocular Hemodynamics,Blood Glucose Level and Visual Impairment in Diabetes Retinopathy

Abstract:Objective:To investigate the correlation between ocular hemodynamics,blood glucose level and visual impairment in diabetes retinopathy(DR).Methods:From September 2021 to June 2024,100 patients with diabetes retinopathy admitted in Puyang Oilfield General Hospital were selected as the research object,and they were set as DR group(165 eyes).Another 100 patients with simple type 2 diabetes without DR were selected as non DR group(169 eyes),and the ocular hemodynamics and blood glucose levels of the two groups were compared.Then 100 DR patients were divided into mild group(n=22,38 eyes),moderate group(n=43,79 eyes),and severe group(n=25,48 eyes)according to the degree of visual impairment of patients.The ocular hemodynamics and blood glucose levels of the three groups were compared,and the correlation between ocular hemodynamics,blood glucose levels and visual impairment of diabetes retinopathy was analyzed.Results:The DR group had higher ocular artery pulsatility index(PI),resistance index(RI),central retinal artery PI,RI,fasting blood glucose,2-hour postprandial blood glucose,and glycosylated hemoglobin than the non DR group.The DR group had lower end diastolic velocity(EDV)and peak systolic velocity(PSV)of the ocular artery,as well as lower central retinal artery EDV and PSV than the non DR group,with statistically significant differences(P<0.05);the PI,RI of the ophthalmic artery,RI of the central retinal artery,fasting blood glucose,and glycosylated hemoglobin were higher in the severe group than in the mild and moderate groups,while the PSV of the ophthalmic artery,EDV,and PSV of the central retinal artery were lower in the severe group than in the mild and moderate groups,with statistically significant differences(P<0.05);the Spearman correlation analysis results showed that the levels of visual impairment in DR patients were positively correlated with ophthalmic artery PI,RI,central retinal artery PI,RI,fasting blood glucose,and glycosylated hemoglobin,the levels of visual impairment in DR patients were negatively correlated with ophthalmic artery PSV,central retinal artery EDV,and PSV,with statistically significant differences(P<0.05).Conclusion:Compared with patients with type 2 diabetes alone,patients with diabetes retinopathy have significant ocular hemodynamic abnormalities and relatively high blood glucose levels,and ocular hemodynamics and blood glucose levels are closely related to the degree of visual impairment in DR patients.
