Abstract:Objective:The effect of observing the position of the pressure sensor on the mechanical axis of the right atrial vein on the postoperative invasive blood pressure monitoring and nursing care of patients undergoing cardiac bypass grafting. Methods:It adopted a prospective approach and included 101 patients who underwent coronary artery bypass grafting at Joint Logistics Support Force 988th Hospital of the Chinese People's Liberation Army from January 2022 to June 2023.They were randomly divided into Group A(40 cases),Group B(31 cases),and Group C(30 cases). All patients underwent invasive blood pressure monitoring postoperatively. With the forearm limb height kept consistent,Group A had the pressure sensor placed at a position aligned with the venous hemodynamic axis of the right atrium.Group B had the sensor positioned at 5 cm(H1),10 cm(H2),and 20 cm(H3)below the venous hemodynamic axis of the right atrium at 8 h,16 h,and 24 h postoperatively, respectively.Group C had the sensor placed at H1,H2,and H3 above the venous hemodynamic axis of the right atrium at the same time points. The blood pressure monitoring results[systolic blood pressure(SBP),diastolic blood pressure(DBP)]and the incidence of adverse outcomes due to blood pressure abnormalities were compared among the three groups. Results:At 8 hours,16 hours and 24 hours after the operation, compared with the patients in group A,the levels of SBP and DBP in group B were lower, while those in group C were higher, the differences were statistically significant(P<0.05).The total incidence of adverse prognosis caused by abnormal blood pressure in group A was lower than that in group B and group C,and the difference was statistically significant(P<0.05).Conclusion:When performing invasive blood pressure monitoring on patients undergoing cardiac bypass surgery, the monitoring results may be affected by the relative position of the right atrial venous mechanical axis and the pressure sensor.Paying attention to the position of the pressure sensor and aligning it with the mechanical axis of the right atrial vein can ensure the accuracy of blood pressure monitoring results, which has a positive significance in guiding clinical treatment and reducing the risk of adverse prognosis caused by abnormal blood pressure in patients.
